UCB wins FDA approval for ZILBRYSQ autoinjector ahead of 2027 launch
The once-daily device adds a second ZILBRYSQ self-injection format for adults with anti-AChR-positive gMG, with availability expected in the first quarter of 2027.

A second ZILBRYSQ self-injection format is coming for adults with generalized myasthenia gravis who are anti-acetylcholine receptor antibody positive after UCB EBRUCB announced U.S. approval of its autoinjector on Friday. Commercial availability is expected in the first quarter of 2027, adding an autoinjector alongside the prefilled syringe for the once-daily treatment.
The FDA's drug record lists an Oct. 15 action under supplement 15 to application 216834, with the updated label unavailable there. UCB says the approved population is adults with generalized myasthenia gravis who are anti-acetylcholine receptor antibody positive.
UCB is a large biopharmaceutical company based in Belgium and developing treatments for neurological and immune-system diseases.
UCB shares were down 0.68 per cent to €204.60 in Brussels at 5:37 p.m. CEST on Friday, according to EBRUCB. The announcement does not disclose an autoinjector price, insurance terms or a revenue forecast.
"For a chronic condition like gMG, having different administration options allows patients to choose the approach that best fits their individual preferences and routines," UCB chief medical officer Donatello Crocetta said in the release.
Delivery Studies Support The New Device
The autoinjector automatically delivers a predefined dose when activated against the skin. ZILBRYSQ, or zilucoplan, was already available in a prefilled syringe for self-administration, so the new device gives patients and care partners another way to administer the same medicine.
The supporting Phase 1 study, DV0012, enrolled 14 healthy adults in an open-label crossover comparison. UCB said it established bioequivalence between administration through the autoinjector and the prefilled syringe, meaning the approval rests partly on comparable drug exposure.
The Phase 3b DV0013 study enrolled 31 adults with generalized myasthenia gravis who self-administered the drug daily. Among 449 autoinjectors used and returned, complete dose delivery was achieved in 99.8 per cent of administrations, according to UCB. That completion rate measures device delivery rather than improvement on a disease-severity scale.
Injection-site pain was the most common treatment-emergent adverse event, reported in three patients, or 9.7 per cent. The release provides no head-to-head symptom-control results against competing medicines.
A separate human-factors study involved 100 participants, including patients, caregivers and healthcare professionals. 90 per cent completed an injection on their first attempt without training; the product's patient website nevertheless says self-administration requires preparation and proper training.
UCB's safety information includes a boxed warning for serious meningococcal infections. It calls for vaccination at least two weeks before treatment unless the risk from delaying therapy outweighs the infection risk, and warns that infection can occur despite vaccination.
Rivals Already Offer Self-Injection
argenx SE EBRARGX NASDAQARGX already offers VYVGART Hytrulo self-injection through a prefilled syringe. In its May 8 approval announcement, argenx said the FDA expanded VYVGART and VYVGART Hytrulo to all adult generalized myasthenia gravis serotypes, giving the rival a broader eligible population than UCB describes for ZILBRYSQ.
UCB also sells RYSTIGGO, a different targeted treatment delivered by subcutaneous infusion. Its indication includes adults positive for either acetylcholine receptor antibodies or muscle-specific tyrosine kinase antibodies, according to Friday's release, giving UCB another treatment alongside ZILBRYSQ.
UCB generated €369 million in operating cash flow during the first half of 2026, according to its July 30 financial update. The autoinjector announcement provides no estimate of additional development spending or sales attributable to the device.
The next commercial milestone is availability in the first quarter of 2027.
